Appointments, Scheduling, and Deposits

Please contact the office in advance to schedule an appointment. 

New patients are required to make a deposit at least two weeks prior to a scheduled appointment as follows:

– A $25.00 deposit is required for children under the age of 18
– A $50.00 deposit is required for young adults and adults over age 18

The deposit can be made via credit card, money order, or cash. The deposit is deducted from the cost of the office visit, and the balance is paid when the patient is seen.

Dr. Burton does not participate in any health insurance plans, including Medicare, Medicaid, Blue Cross/Blue Sheild, Personal Choice etc.  

Check with your insurance company to ascertain if they will reimburse you for “out-of-pocket” medical expenses, since Dr. Burton is out of network.  

In most cases, you must meet their out of network deductible BEFORE you are entitled to be reimbursed for medical expenses. 

The laboratory that the center uses for blood work does accept certain major insurance carriers.

New Patient Visit

Duration: Up to 60 minutes

Cost: Contact the office for fees

Includes: Full review of medical history, physical examination, review of previous laboratory studies, previous medical reports/imaging, consultation regarding current medications/supplements, medical consultation regarding presenting condition, personalized treatment regime. 

 

New Patient Registration Form

NOTE:  New patients are not able to be seen without contacting the office to complete an essential intake process.  

Once the intake process is complete, an appointment is scheduled, and the deposit is received, please download and print the New Patient Registration form.  

Please bring the completed registration form with you to your initial visit or fax the form to (610) 649-4325.
